Unity Family Hospice is a Medicare-certified program sponsored by St. Gabriels Hospital. It is composed of professionals and volunteers concerned with helping individuals and their families face a terminal illness. The program offers physical, emotional and spiritual comfort to both clients and families. Hospice provides care to those people who are dealing with the natural feelings, reactions, and problems accompanying death. Services Provided An interdisciplinary team of physicians, nurses, social worker, clergy, and trained volunteers aid in the care and support of the client and family. Consultants like nutritionists, pharmacists, therapists and others, also help when needed. The team assists the client and family to receive the level of care appropriate to their needs. Each level of care is carried out under the direction of a physician and includes the following: Regular home care visits by qualified hospice staff and volunteers. Continuous nursing care at home for short periods of crisis. Short-term inpatient care to provide respite for caregiver. Short-term inpatient care for acute or chronic symptoms management. Home health care services provide many benefits to the client. They enable acute care services to be delivered to people in need in their own homes at a fraction of the cost of hospitalization. Unity Family Home Care Offers several additional advantages. Access to more acute care services such as laboratory tests, IV therapy, Physical therapy, wound care and post-hospitalization care. Services provided within 24 hours of discharge, if needed.
