Senior Outreach and Consulting, a program for senior citizens and their caregivers, is part of the Amherst H. Wilder Foundation, a nonprofit health and human services organization that has served greater St. Paul, Minn., since 1906. The Wilder Foundation operates dozens of programs that help children succeed in school, older adults remain independent, troubled youth create healthy futures, and individuals and families maintain long-term housing. The program s team of licensed gerontological social workers provide care management and caregiver consultation services in partnership with Eldercare Partners. Its caregiver services offer personalized services that enhance knowledge and emotional skills of family caregivers. The Senior Outreach and Consulting program also provides education and training opportunities and manages caregiver challenges in family system. The program s caregiver services are funded by a grant from the Metropolitan Area Agency on Aging (MAAA) with funds from the federal government s Older Americans Act. Its care management and consultation services provide assistance in several issues, such as housing decisions, community resource planning, financial and legal issues, and dementia and memory loss.
