HistoryFamily Health Centers was established by the Louisville-Jefferson County Board of Health in 1976 with a freestanding Board of Governors. Three years later, Family Health Centers received its first federal grant under Section 330 of the Public Health Service Act, which established the organization as a federally qualified health center (FQHC). On March 14, 1988, Family Health Centers (FHC) opened the St. John Health Center for the Homeless. Located in Louisville's only homeless day shelter, this federally funded project (Section 340, Public Health Service Act) provided primary health care and substance abuse services to homeless adults and children living in area family emergency shelters. On February 28, 2000, the center moved into a new outpatient facility adjacent to the existing center at 712 East Muhammad Ali Boulevard. Along with the relocation came a new name, Phoenix Health Center.SpecialtiesFHC-Phoenix provides primary care services, behavioral health care, case management, on-site pharmacy and dental office, substance use assessment and referral to treatment services, health insurance enrollment assistance, permanent housing and a range of supportive services for indivudals who are homeless.
