Phoenix Health Center
HistoryFamily Health Centers was established by the Louisville-Jefferson County Board of Health in 1976 with a freestanding Board of Governors. Three years later, Family Health Centers received its first federal grant under Section 330 of the Public Health Service Act, which established the organization as a federally qualified health center (FQHC). On March 14, 1988, Family Health Centers (FHC) opened the St. John Health Center for the Homeless. On February 28, 2000, the center moved into a new outpatient facility adjacent to the existing center at 712 East Muhammad Ali Boulevard. Along with the relocation came a new name, Phoenix Health Center.SpecialtiesFHC-Phoenix provides primary care services, behavioral health care, case management, on-site pharmacy and dental office, substance use assessment and referral to treatment services, health insurance enrollment assistance, permanent housing and a range of supportive services for indivudals who are homeless.
