Weiss Pediatric Care is proud to be certified as a Level 3 Patient-Centered Medical Home by the National Committee of Quality Assurance (NCQA). The primary care medical home is accountable for meeting the large majority of each patient's physical and mental health care needs, including prevention and wellness, acute care, and chronic care. This model is relationship-based with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patient's unique needs, culture, values, and preferences. Recognizing that patients and families are core members of the care team, our medical home practice ensures that they are fully informed partners in establishing care plans.

Weiss Pediatric Care delivers the next generation of health care for children and young adults by offering over fifty years combined experience in pediatric medicine, child development and parenting education. Founders, Dr. Robert Weiss and Diane Weiss, M.S., have been nationally recognized throughout their careers for providing outstanding care to families, leading their respective fields through education, and remaining on the cutting edge of knowledge and practices. Our single-physician practice allows us to combine the more intimate, comprehensive and personal doctor-patient relationships of the past, with the benefits of our modern medical system. At Weiss Pediatric Care, we believe that to grow healthy children from newborns to young adults, it takes much more than just checkups and sick visits.
