The San Francisco Community Clinic Consortium, also known as SFCCC, is a nonprofit health care system that operates various community-based medical clinics. Established in 1982, the system provides primary care services to more than 70,000 patients each year. It offers childhood immunization and substance abuse treatment services. The San Francisco Community Clinic Consortium offers the Health Care for the Homeless program, which focuses on providing health care, housing assistance and social services to homeless people. The system offers mental health care, HIV counseling and referral services. Its clinics in partnership with the San Francisco Department of Public Health centers provide centralized program design and management, data collection, reporting, and technical assistance and training services.
