Community Hospice is a nonprofit organization that provides care to patients and their families in private homes and skilled nursing facilities. It has a team of registered nurses, dieticians, social workers, licensed vocational nurses and home health aides. It offers a variety of medical and psychological services, such as pain and symptom management, coordination of all health care providers, wheelchairs, emotional support, short-term counseling, and referral to community counseling and support agencies. It also offers services that include spiritual counseling, work with clergy of patient's choice, and church and synagogue support. It also offers volunteer services that include companionship, emotional support and practical caregiver support. Additionally, it offers a wide range of therapies, such as physical, occupational, speech and dietary. Community Hospice was established in 1979, and is located in Modesto, Calif.
